Direct Services


Central Intake

The Family, Aging, & Disability Resource Center serves as the central point of entry into WACOG Area Agency on Aging services system for older adults and persons with disabilities in La Paz, Mohave, and Yuma Counties. Our Central Intake Specialist connect families and individuals to WACOG’s direct and contracted services or other information, social services, and supports needed. 

Case Management

Our Case Managers conduct a comprehensive assessment to determine the client’s level of need and eligibility for WACOG services. Upon being deemed eligible, an individual care plan is developed and referrals to appropriate services are made to ensure clients’ needs are being met. The client’s needs are reevaluated on a regular basis and the services provided are updated accordingly.    

Family Caregiver Support Program

The purpose of the Family Caregiver Support Program is to provide support to family caregivers providing care to a person over 60. Types of support include: respite care, peer counseling, outreach, community education and information, and caregiver training.

Long Term Care Ombudsman

The Long Term Care Ombudsman program provides support and advocates for residents of Long-Term Care and Assisted Living facilities to assure that individual rights are protected and resident’s needs are met. Our Ombudsman work to resolve issues of individual residents and advocate to improve the quality of life and care of residents.  

State Health Insurance Assistance Program (SHIP)

The State Health Insurance Assistance Program offers benefits counseling and assistance with questions regarding Medicare, Arizona Health Care Cost Containment System (AHCCCS), Arizona Long Term Care (ALTC), supplemental insurance, etc. The SHIP program intends to educate, advocate, counsel, and empower individuals to make informed decisions about their healthcare options.   

WACOG Rider Assistance Program (WRAP)

The WACOG Rider Assistance Program is a mileage reimbursement program designed for elderly passengers (60+) who rely on others for transportation to destinations that include medical appointments, grocery stores, and the pharmacy. Mileage is paid to the WRAP participant at a rate of $0.37 per mile for the purpose of reimbursing the driver of the participant.

Western Arizona Council of Governments, Area Agency on Aging Elder Rights Unit is comprised of the State Health Insurance Assistance Program (SHIP) and Long Term Care Ombudsman.

State Health Insurance Program

(SHIP) Staff and volunteers are trained and available to assist with concerns and questions about Medicare benefits, AHCCS, ALTCS (Medicaid), supplement insurance, HMO's and the like. For additional information and assistance in Mohave County call: 753-6247; in Yuma County call: 782-1886. SHIP (State Health Insurance Program) Counselors can be reached toll free at 1-800-782-1886.

Ombudsman Staff and volunteers are certified to provide support and advocacy for residents of Long-Term Care and Assisted Living facilities to assure that individual rights are protected and needs are met.
