Contracted Services
The following services are contracted with local agencies by WACOG Area Agency on Aging:
Adult Day Care
Provides supervision during the day for adults in a safe supportive environment.
Case Management
The case manager is considered the 'gatekeeper' for most services by determining the client's needs and eligibility. The case manager for all homebound clients performs a complete physical and cognitive assessment and develops a care plan for the individual. Unmet needs are identified and referrals to appropriate services are made. The client's needs are re-evaluated on a regular basis and the services provided are updated at that time.
Specifically related to the Family Caregiver Program, this service is to be provided to caregivers of persons age 60 years or older and grandparents or older relative caregivers of children (under 19 years of age). Caregivers are considered to be the client. For other Home and Community Based Services, the person in need of care is considered the client.
Home Adaptation
Provides for safety, structural repairs and/or modifications to the home.
In Home Services
Trained staff perform non-medical personal care tasks in the home of the individual. These services include: Home Health Aide, Housekeeping, Personal Care and Visiting Nurse.
Legal Services
This service is provided to clients who meet eligibility requirements and have no resources to employ an attorney. Volunteer legal advocates can provide certain aspects of legal assistance.
Nutrition Programs
Provides nutritious meal (1/3 RDA) in either a Congregate or *Home Delivered Meal setting.
Respite
Provides temporary relief to the individual's primary caregiver for a specified period of time. This service may be provided in the home or in a group setting.
Transportation
Services designed to provide access to needed community resources to include medical appointments and to the congregate meals program.
